In a recent development, health officers from the Biden administration encouraged an international medical experts group to remove age restrictions on body-modifying surgeries for minors struggling with gender dysphoria. These insights emerge from fresh emails released by the World Professional Association for Transgender Health (WPATH), a leading LGBT health non-profit organization.

These email extracts pinpoint an effort made by staff working under an eminent trans-identified leader in the U.S. Department of Health and Human Services, who successfully persuaded WPATH to eliminate recommended limits for transgender surgeries. The emails deal primarily with Rachel Levine, the Assistant Secretary for Health at the U.S Department of Health and Human Services, who is openly transgender.

Legal filings linked to a federal lawsuit challenging Alabama’s ban on transgender surgeries and puberty blockers for minors surfaced these email excerpts. Last year, WPATH introduced Standards of Care guidelines that surprisingly omitted sections about minimum age requirements for children seeking puberty blockers, cross-sex hormones, or body-altering surgical procedures. The group asserted defense against potential lawsuits as its motivation behind the removal.

The draft guidelines initially proposed reduced age restrictions — 14 years old for hormonal treatments, 15 for mastectomies, 16 for breast augmentation or facial surgeries, and 17 for genital alterations or hysterectomies. A WPATH member cited Sarah Boateng — then-chief of staff to Levine who later assumed duties as Principal Deputy Assistant Secretary for Health in October 2022 — expressing fears over specific regulations causing harm to transgender care.
